Medi-Cal's Assisted Living Waiver has more people waiting than enrolled. How it works in San Diego County, and the rungs that work while you wait.

In February 2026, 20,262 Californians were waiting for Medi-Cal's Assisted Living Waiver, more than the 14,809 enrolled. In June 2024 the line was 3,049. Over the same stretch, enrollment slipped from its 16,294 peak in November 2024 to 14,809: about 1,485 fewer people in the program while the line grew more than sixfold. (March 2026: 15,910 waiting.) San Diego is one of the 15 waiver counties; the State doesn't publish a county breakdown.

How the waiver works in San Diego

  • What it pays: Medi-Cal pays for the care in an assisted living home (a residential care facility for the elderly or an adult residential facility) or in public subsidized housing, at one of five levels: $95.69 to $270.80 a day on the State's rate sheet (updated December 2025).
  • What it doesn't pay: room and board. The resident pays the rent; the State says most use their SSI/SSP check. (Its 2024 example: room and board of $1,398.07 on SSI income of $1,575.07.)
  • Who qualifies: on Medi-Cal with no share of cost, needing the level of care a nursing home gives, and able to live safely in assisted living or subsidized housing in a waiver county.
  • How to get in line: call a Care Coordination Agency that serves San Diego County from the State's list. One of its registered nurses assesses your parent. The State releases open slots to the agencies on a regular basis.

Who gets the openings

Enrollment is below the federal limit: the waiver allows 16,344 people at a time in its second year and 18,144 in its third, and 14,831 were enrolled in March 2026. What decides who gets in is the order openings go out. Each month the State releases them to the agencies in a set split: half to people who have lived in a nursing home or other institution for at least 60 days, a quarter to Adult Protective Services referrals, and a quarter to Long-Term Care Ombudsman referrals. Applicants from home get openings only if any remain, and since June 22, 2026 an Ombudsman referral requires living in a skilled nursing facility with a discharge notice.

If your parent is in a nursing home or rehab now, the 60-day mark matters: ask the facility's social worker about the waiver before discharge. If your parent is at home, get on the list anyway and lean on the rungs below. Every other senior line in San Diego: the waitlists, side by side.

While you wait: the rungs that work now

  1. PACE. The same nursing-home level-of-care test, but care comes to the home and a day center, with no premium on Medi-Cal.   2. The Medi-Cal plan's assisted-living support. One of Medi-Cal's "Community Supports" helps people move to or stay in assisted living instead of a nursing home (the County calls it Nursing Facility Transition/Diversion to Assisted Living). San Diego has all 14 standard Community Supports live (DHCS, January 2026).   3. IHSS if your parent is staying home for now: the County can pay for help at home, and a family member can be the caregiver. How IHSS works.
  4. Adult day health care (CBAS): nursing, a meal and a ride, days only. AmeriCare in San Marcos, 760-682-2424.
  5. A break for the family caregiver: respite money and counseling from the Southern Caregiver Resource Center, no income test, 800-827-1008.
  6. Veterans and surviving spouses: VA Aid and Attendance (up to $29,093 a year for a veteran alone) can help pay for assisted living privately. Free help: 1-866-726-8831.
  7. Protect the Medi-Cal. A share of cost rules out the waiver, so answer every renewal letter. The asset limit for 65+ is $130,000 through June 30, 2027, then $21,000. The Medi-Cal route.
  8. Shortlist homes now. Look up any facility's license and inspections on the State's care facility search, and ask each one whether it takes waiver residents.
