
Several of the programs that keep an older parent out of a nursing home in San Diego have a line, and some lines are closed. Here is every one we could size from official numbers: how long it is, who gets in first, and the number to call. Then the help you can start this week while you wait.

Counts are the latest the State, County and cities publish. No program publishes who is on its list, and neither do we.
How long the longest lines are
Home and Community-Based Alternatives waiver: "up to 2 years" (San Ysidro Health). Carlsbad: "approximately a 15 year wait" (City of Carlsbad). County Housing Authority: an average of 16 years for those selected (FY 2025-26 Agency Plan).
Who gets in first
For the assisted living waiver, the State releases openings to the agencies each month in a set split: half to people who have lived in a nursing home or other institution for at least 60 days, a quarter to people referred by Adult Protective Services, and a quarter to referrals from the Long-Term Care Ombudsman. Applicants from the community get openings only if any remain. Since June 22, 2026, an Ombudsman referral also requires living in a skilled nursing facility with a discharge notice for a lower level of care.
Two things follow. If your parent is in a nursing home or rehab now, the 60-day mark matters: ask the facility's social worker about the waiver before discharge. If your parent is at home, the line is a long shot, so get on it (it's free) and build the plan from the help you can start now.
Help you can start now
- PACE: no published waitlist; call one of four North County programs to start; no premium with Medi-Cal. Details on the care ladder.
- IHSS: no waitlist, and a family member can be the paid caregiver. How it works.
- The Medi-Cal plan's Community Supports: respite, personal care, home changes and assisted-living support; San Diego has all 14 standard ones live.
- Caregiver respite: Southern Caregiver Resource Center, no income test, 800-827-1008.
- Veterans: VA Aid and Attendance, up to $29,093 a year for a veteran alone; free help at 1-866-726-8831.
- A guy for the in-between: errands, groceries, forms. In the County's 2024-2028 plan for older adults, its own staff noted "a need for more affordable services for domestic tasks, minor home repairs, and garden upkeep for those who do not qualify for IHSS."
