Does my Medicare work when I winter here?

For snowbirds who keep their home and coverage in another state: what follows you to San Diego, what doesn't, and what to ask before Open Enrollment.

It depends on which Medicare you have. If you keep your home and coverage in another state and spend the winter here, Original Medicare follows you; a Medicare Advantage plan mostly doesn't. Medicare's own advice: "If you live in another state for part of the year, check if the plan will cover you there."

How to tell: if you show your red, white and blue Medicare card at the doctor, you likely have Original Medicare. If you show an insurance company's card instead, it's likely a Medicare Advantage plan.

Original Medicare (plus a supplement and a drug plan)

  • "You can use any doctor or hospital that takes Medicare, anywhere in the U.S." Ask San Diego doctors whether they accept assignment; Medicare won't pay providers who opted out, except in emergencies.
  • Medigap follows you: "You can keep your current Medigap policy no matter where you live, as long as you still have Original Medicare." The exception is a Medicare SELECT policy, which uses a network.
  • The drug plan is the one to check: some plans only cover drugs at in-network pharmacies. Ask which San Diego pharmacies are in network, or get a 2- or 3-month supply (or mail order) before you leave.

Medicare Advantage

  • Away from home, the plan must cover emergency care, out-of-area urgent care and temporary dialysis.
  • Routine care (checkups, specialists, refills) depends on the plan: an HMO generally covers only its network; a PPO usually covers out-of-network care at a higher cost; some plans add a visitor or travel benefit.
  • The 6-month rule: if you're away from the plan's area for more than 6 months in a row, the plan must disenroll you, unless it has a visitor/traveler benefit, which can stretch that to 12 months. Mail returned as undeliverable can count as being away, so keep the plan's mail forwarded.

Consecutive months away before the plan must disenroll you (42 CFR 422.74 and 423.44). Original Medicare and Medigap have no such limit inside the U.S.

Thinking of switching for 2027?

Open Enrollment runs October 15 to December 7, 2026, for coverage starting January 1. You can switch from Medicare Advantage to Original Medicare then. The catch: that doesn't give you a federal right to buy a Medigap policy. Outside your one-time Medigap enrollment window (or a 12-month trial right if you just tried Advantage for the first time), an insurer can review your health and say no.

  • Illinois: if you're 65 to 75 and already have Medigap, you get 45 days from your birthday each year to switch to equal or lesser benefits with the same insurer or an affiliate (state law, current version since January 1, 2026).
  • California has a similar rule: a 60-day window starting on your birthday to move to an equal or lesser Medigap plan.
  • In an Advantage plan already? January 1 to March 31, 2027, you can change plans or go back to Original Medicare.

Moving here for good

Moving out of an Advantage or drug plan's area opens a Special Enrollment Period: it starts the month you move (or the month before, if you tell the plan first) and runs 2 full months after. If you don't pick a new Advantage plan, you're moved to Original Medicare. Moving out of the plan's area also brings a guaranteed right to buy certain Medigap plans if you switch to Original Medicare. The rest of the move: moving a parent to North County.

Ask the plan these five questions

  1. Is San Diego County covered in 2027, through your service area or a visitor/travel benefit? For how long, at what cost, and do I register?
  2. What routine care do you cover while I'm in California, and does it need prior approval?
  3. Which San Diego pharmacies are in network for my drugs, and will you mail 90-day supplies to my California address?
  4. How do you count the 6 months away, and what address settings keep your mail from coming back?
  5. If I switch to Original Medicare, would I have a guaranteed right to buy Medigap, or would I be medically reviewed?

Near the border: Medicare drug plans don't cover prescriptions bought outside the U.S., and Medicare usually doesn't cover care abroad. Change your official address with Social Security, not Medicare.
