PACE in North County: the four programs

One team for doctors, home care, meals, therapy, a day center and rides, so a parent who needs nursing-home-level care can stay home.

PACE (Program of All-Inclusive Care for the Elderly) is a Medicare and Medi-Cal program where one team provides all of a person's care. Four programs serve North County San Diego, and all four take Solana Beach (92075) and Cardiff (92007) on the State's July 2026 PACE ZIP code list.

Who qualifies

  • Age 55 or older.
  • Lives in a PACE service area.
  • The State finds they need nursing-home level of care.
  • Can live safely at home or in the community when they enroll.

What it costs

  • With Medi-Cal: no monthly premium.
  • Medicare only: a monthly premium.
  • Either way: Medicare says there's no deductible, copay or coinsurance for any drug, service or care the PACE team approves. Care the team didn't approve can be billed to you.

How to start

Call any of the four. An enrollment specialist does a home assessment, and the State approves the level of care. Some programs help with the Medi-Cal application. The State has paused new PACE organizations and service-area expansions since November 20, 2025, for at least two years; that doesn't stop anyone from enrolling in the programs already here.
